Cornerstone

Cornerstone Care WA Pty Ltd is a newly established NDIS provider built by professionals with years of experience across disability, advocacy and community services. We are building something intentionally from the ground up — because quality support deserves strong foundations.

A note from our founders
"We have both worked in disability and community services long enough to know what good support looks like — and to recognise when it is missing. Participants being treated as tasks to tick off. Workers rotating every week. Support plans that did not reflect anyone's actual life. Families left in the dark. We started Cornerstone Care WA because we believed we could build something better. Something built properly, from the ground up. That is what we are doing."

What we offer

Supports that fit your life

We do not offer a menu you pick from. We work with you to figure out what actually helps and then we deliver it properly.

Personal Care and Daily Living

Hands-on support with bathing, grooming, dressing, personal hygiene, continence care, eating assistance, and household tasks like cleaning, laundry and meal preparation. Always delivered with dignity and at your pace.

Community Access and Participation

Support to get out and be genuinely part of your community — social outings, events, recreational activities, cultural programs and creative participation. We also provide safe, reliable transport to appointments, activities, work or social events, and support participants to build independent travel skills on public transport where that is a goal. Your worker is there to support you, not participate for you.

Life Skills and Independence

Building capacity in everyday life — cooking, budgeting, shopping, managing routines, using public transport, communication skills. Also supports participants through major life transitions like leaving school, moving out, or adjusting to a change in needs.

Shared Living Support

Support for participants living in a shared home with others — household tasks, cooking, cleaning and laundry in that shared environment, while also supporting positive relationships between housemates and the coordination that comes with shared living.

Support Coordination

Help to understand, navigate and make the most of your NDIS plan. We connect you with the right services, help set up your supports, work through any challenges along the way, and make sure your plan is actually working for your life — not just sitting in a folder.

Understanding the NDIS

New to the NDIS or still finding your feet? Here are the four things people ask us most, written plainly and without the jargon.

What is the NDIS?

The National Disability Insurance Scheme is a government program that funds supports and services for Australians under 65 who have a permanent or significant disability. It gives you a plan with funding you can use to access the support you need to live your life — on your terms.

What does an NDIS plan include?

Your plan outlines your goals and the funding allocated to help you achieve them. Funding is split into categories — Core Supports for everyday activities, Capacity Building for developing independence, and Capital Supports for equipment or home modifications. Not every plan includes all three, and that is okay.

How do I know what I am entitled to?

Your funding is based on your disability, your goals, and what the NDIS considers reasonable and necessary. It can feel unclear at first — and that is completely normal. A Support Coordinator or plan manager can help you work through it, and we are always happy to point you in the right direction in a general sense.

Can I change providers?

Yes — and this is important to know. You have the right to choose and change your providers at any time. If your current support is not working, the fit is wrong, or you simply want something different, you are entitled to make a change. Good providers will never make you feel stuck. We certainly would not.
